Hemodynamic alteration in diabetic nephropathy measured by renal Doppler ultrasonography: does it predict the outcome of the disease?

چکیده
We read the article by Dr. Futrakul et al. with interest [1]. They searched for a parameter that was more sensitive than microalbuminuria for early detection of diabetic nephropathy and compared creatinine clearence, fractional excretion of magnesium and intrarenal hemodynamics by renal scintigraphic evaluation. For this purpose, they calculated peritubular capillary flow, efferent arteriolar resistance, glomerular hydrostatic pressure and filtration coefficient of glomerular capillary with 99mTc-labelled DTPA (diethylene triamine pentaacetic acid) and 131I-labelled hippuran renal scintigraphy. Accordingly, we have designed a study to examine the intrarenal hemodynamic abnormalities in different stages of nephropathy and further measured the intrarenal resistive index (RI) and pulsatility index of interlobar arteries detected by renal Doppler ultrasonography (USG) for each kidney. For our ongoing prospective randomized trial, we included 20 healthy, non-diabetic patients as a control group; 26 type-2 diabetic patients with microalbuminuria and 19 type-2 diabetic patients with albuminuria as study groups. The clinical characteristics of the control and diabetic subjects with their mean RI values are reported in Table 1.

Comparison of Diagnostic Value of Intrarenal Doppler Indices and Microalbuminuria for Detection of Diabetic Nephropathy in Type II Diabetic Patients
Background & Aims: Diabetes is the most common cause of renal insufficiency throughout the world. The main cause of renal failure in diabetic patients is microvascular endothelial injuries that is secondary to atherosclerosis and ischemia. The efficacy of renal Doppler indices such as resistive index (RI) and pulsetile index (PI) for the evaluation of intrarenal arteries has been shown. This st...
متن کاملRole of renal resistive index in unilateral acute renal obstruction, diabetic and hypertensive nephropathy
Background: Variation in Renal restive index (RI) values measured using duplex Doppler ultrasonography has been shown to correlate with the degree of renal impairment in acute renal obstruction ,diabetic and hypertensive nephropathy patients. Aims and objectives: The primary objective of this study was to determine whether the intrarenal resistive index can be used as a predictor of progression...
متن کاملRelationship between Intra-renal Arterial Resistance Index (RI) and Albuminuria in Diabetic Patients
OBJECTIVE: Diabetic nephropathy is a major diabetes complication. Arterial resistance index (RI) may predict deterioration in kidney function. This study was designed to compare renal arterial RI in different stages of renal function according to glomerular filtration rate (GFR), serum creatinine level and proteinuria. MATERIALS AND METHODS: In a cross-sectional study on 81 diabetic patients i...

Diabetes mellitus is the most common cause of chronic renal disorders and end-stage kidney disease in developed countries. It is the major cause of dialysis and transplantation. Failure in renal function causes wide disorders in the body. Diabetes results in wide range of alterations in the renal tissue. It is believed that early histological changes in diabetic nephropathy are detectabl...

The aim of this study was to determine the effects of intramuscular injection of medetomidine on intra-renal arteriesresistive and pulsatility indices by duplex Doppler ultrasonographyin clinically normal adult domestic shorthair cats. For this purpose, twenty-six neutered adult healthy domestic shorthair cats (13 females and 13 males) were evaluated. B-mode, color Doppler and pulsed wave Doppl...
